A customized service plan, as determined by our Assessment
Program, will establish your level of care and corresponding
monthly fee.
A Customized plan, to meet your individual
needs, can also be established for a husband or wife residing in
an adjoining apartment, or an additional person in the same
apartment.

change with 30-day written notice.
